- Complete an evaluation for each request approved
- Include specific details describing how the child benefited from the experience or item.
- Share the child’s key comments on the experience and how it made a difference. Use the child’s own words. For example, Stephen took swimming lessons. He now says, "I'm not scared of the water. I'm not scared of anything anymore."
- If educational assistance was received, please state the subject and the scholastic difference before and after the tutoring. For example, Maria was previously reading at a third grade level; now she is reading at a fifth grade level and enjoying reading.
